Collect and keep your validated deposit slip.

Ensure that your name is written clearly on the validated deposit slip.

Complete the Endometriosis Walk registration form below.

Select bank deposit & deposit slip upload for payment method.

Upload a copy of the validated deposit slip in the field entitled validated deposit slip.

Click the register button to submit the form and complete registration.

Ensure receipt of message confirming successful registration.

Persons on our supplemental list will be contact on Friday 29 March, 2019 if the requisite sizes become available. They will be allowed to make payment and register on the day of the walk during the hours 1pm -2pm.

Bank Deposit Details
Interested persons may deposit the T-shirt fee at any branch of the First Citizens Bank nationwide to the account of the Trinidad and Tobago Endometriosis Association, acc. no.2178820. A copy of the validated deposit slip clearly showing the person's name must be retained and presented when collecting the T-shirt.
